Term Enrollment Reporting Form

This form should be used when reporting changes in your enrollment status. The Student Financial Aid and Scholarships Office (SFA) anticipates all students will be enrolled full time (12+ credit hours for undergraduate; 8+ credit hours for graduate), unless otherwise notified via the SFA Summer Financial Aid Application or by individual correspondence with the SFA Office. Submit this form to notify SFA of your current enrollment status.

Note: Financial Aid will not process this request until your submitted enrollment change matches your actual enrolled hours. Wait-listed classes do not count as enrolled hours.
